What do I do if my certificate is lost, damaged, or incorrect?
If you began your course before May 1st, 2022, and lose your certificate, you can contact our office to request a duplicate by calling 734-665-7374. The cost for a duplicate certificate is $15. The State of Ohio also requires a Duplicate Request form to be filled out and sent to us before processing. We will need to collect that fee and form before the new certificate is made and mailed.
